You'll need

1. Class Members who submit valid claim forms are entitled to choose one of the following Settlement benefits: a Fixed Cash Payment of $150.00 (no documentation is required) or Reimbursement of a maximum of $300.00 for lost time and out-of-pocket expenses (supporting documentation is required). (Deadline: November 16, 2026)
2. If you wish to object to the Settlement, you must file your objection in writing with the Clerk of the Court, and send a copy of your objection to the attorneys for all Parties. (Deadline: November 16, 2026)
3. To exclude yourself from the Settlement Class, you must inform the Settlement Administrator in writing of your name, address, and your intention to be excluded. (Deadline: November 16, 2026)
