Quick steps

1. You must submit your Claim Form online no later than Thursday, December 3, 2026, or mail your completed paper Claim Form to the Settlement Administrator so that it is postmarked no later than Thursday, December 3, 2026.
2. You must complete and mail your written request for exclusion to the Settlement Administrator, postmarked no later than Thursday, December 3, 2026.
3. You must file your objection(s) and/or notice of intent to appear at the Final Approval Hearing with the Court and serve copies to Class Counsel and Defendant's Counsel so that it is postmarked no later than Thursday, December 3, 2026.
4. To exclude yourself from the settlement, you must mail or otherwise deliver a letter (or request for exclusion) stating that you want to be excluded from the L.C. v. Fertility Centers of Illinois, PLLC, Case No. 1:25-cv-02049 settlement. (Deadline: DECEMBER 3, 2026)
5. To object, you must file with the Court a letter or brief stating that you object to the settlement in L.C. v. Fertility Centers of Illinois, PLLC, Case No. 1:25-cv-02049 and identify all your reasons for your objections (including citations and supporting evidence) and attach any materials you rely on for your objections. (Deadline: DECEMBER 3, 2026)
